Community Based Services



Inner Harbour operates a community based services program providing mental health and substance abuse/education services. This program serves youth up to age 21 who are eligible for Core State Contracted/Medicaid services and who have a psychiatric and/or substance abuse diagnosis. Clinical services are provided to clients and families who live in the community as well as to residents in the Inner Harbour RBWO (Room and Board and Watchful Oversight) group setting. The program incorporates the Matrix Model and Dialectical Behavior Therapy (DBT), which are both evidence based.

LOCATION:


  • Youth and families are served through the program at 8341 Grady Street, Douglasville, Georgia 30134.
  • Services are primarily offered in the vicinity of Douglas County; however there is no geographic limitation to the extent that transportation is possible. In addition, services are provided in the clinic and in the community.


STAFFING:


  • Teams are supervised by a licensed Clinical Services Manager who provides clinical supervision, oversight, and coordination of service delivery as well as some direct clinical services.
  • Psychiatrists, psychologists, therapists, substance abuse professionals, community support specialists (paraprofessionals) and nurses are available to provide services as needed.


SERVICES:


  • Initial Assessment is performed and referrals are made to appropriate levels of care. A Nurse provides a physical assessment and conducts an evaluation of need to refer for a physical or psychiatric exam by a physician. Assessment also includes mental status, psychiatric, medical, and substance abuse history of client and family.
  • Psychiatric evaluation/medication management is provided by a psychiatrist for clients in need.
  • Psychological Evaluations provided by a licensed psychologist upon referral from the program to assess intellectual, educational, personality, and/or psychological functioning.
  • Crisis Intervention: In clinic and out of clinic response to crisis is available 24 hours/7 days per week through an on call system.
  • Individual Therapy: Explores issues which impact client's self-esteem, coping/ functioning. For clients with substance abuse issues, also addresses client's history of using, consequences of using, previous attempts to stop using, powerlessness, and willingness to commit to drug free lifestyle.
  • Family Therapy: Explores family dynamics, roles, history, environment, challenges/stressors, and strengths/supports.
  • Multifamily Therapy: Addresses family communication skills, provides education on how to handle relapse if it occurs, provides support among families who often feel isolated by the problems, and addresses family relapse prevention.
  • Group Therapy: Addresses interpersonal communication skills, self-esteem, and coping. For clients at risk for addiction, provides education about 12 steps, relapse prevention, and provides peer support for drug/alcohol free lifestyle.
  • Trauma Group: Serves clients who have experienced abuse: physical, sexual, and/or emotional.
  • 12 Step Group: Provides support to process and complete the 12 steps of recovery based on AA principles.
  • Aftercare Group: Provides continuing support after discharge to prevent relapse.
  • Anger Management Group: Assists clients to develop self-control over thoughts and actions and healthy emotional expression.
  • Coping Skills/Assertiveness Training Group: Assists clients in saying "no" to drugs/alcohol, dealing with peer pressure and family stress, and in developing social and problem solving skills.
  • AA/NA Groups: Clients are referred to AA/NA meetings in the community as appropriate to their needs.
  • Urine Drug Screening: Clients/Guardian sign consent for clients to submit to urine drug screening. Drug screens are utilized to monitor progress of treatment efforts and to assess program efficacy. The program policy will be followed in the administration and reading of test results.
  • Community Support Individual (CSI): Assists client in the development of interpersonal, community coping and functional skills (including adaptation to home, school and healthy social environments).
